All equipment from FlexEquip must be prescribed by a treating health professional. This could be your occupational therapist, speech pathologist, physiotherapist or other suitable professional.
Five easy steps:
- Complete our online intake form and we will send you a Service Agreement to sign
- Tell your health professional you have joined FlexEquip
- Have your equipment needs assessed by your health professional
- Your health professional completes a referral on the FlexEquip website for all the items you need
- FlexEquip arrange delivery and deliver items to you.
